Living Proof Praise Team NJ/NY Trip Itinerary
Price per Student: $25 ($20 Fuel/toll/Parking, $5 For Sunday Breakfast, bring your own spending
money about $30)
7:30am We leave Sabbath morning from TA
11am We sing for Church at the Berean SDA Church 828 Sanford Avenue, Newark NJ, 07106.
1:30pm, They feed us for lunch.
3pm We perform afterwards for an AYS program
6pm Dinner at the Church
7pm Social event in NY
11pm Curfew in my Mother in laws house and Cousins house
10am Depart for touring of the Empire State Building and then Shopping.
5pm Leave for home.
10pm Arrive back at TA

NJ/NY Living Proof Trip Permission Slip

permission to go to NJ/NY with Pastor Hayden and the Living Proof Praise Team leaving TA on
March 6, Sabbath morning at 7:30am and returning Sunday night by 11pm. I give him permission to
seek medical attention for my child if needed and I can be contacted at ________________________
if neccessary. I agree to pay $25 by Friday March 5th or pay $28 via paypal to The Gospel
