PART-TIME YOUTH APPLICATION FORM
I would like to sign up to be a part-time Youth Coach from September 2006- September
2007 and I agree to fulfill, to the best of my abilities, the following TWO responsibilities
below:
Please circle at least TWO numbers below:
1. Attend at least ONE Youth Church service a month.
2. Attend ONE social per quarter.
3. Attend TWO Outreach programs per year.
4. Identify and participate in ONE ministry for the year in Section I.
5. Join ONE Committee planning a Big Annual Programs in Section II.
If you circled number 4 or 5 please continue with Section I or Section II.
If you didn’t circle 4 or 5 please skip to Section III.
Section I: Please circle at least one and at the most two below:

[Youth Explosion, Friends Day, Week of Prayer, or Youth Retreat]

_________If I cannot attend, or if I’m running late, I agree to call Pastor Hayden or a
Youth Coach attending or in charge at the time.

Praise SDA Church beside the Youth Pastor, Melvyn Hayden III, for the saving of young
souls in the kingdom of Jesus Christ.
