YOUTH COACH APPLICATION FORM
I would like to sign up to be a full-time Youth Coach from September 2006- September 2007
and I agree to fulfill, to the best of my abilities, the following 6 criteria below:
1. Attend monthly meetings faithfully, every 4th Sabbath afternoon, after Youth Church.
2. Attend at least ONE Youth Church service a month.
3. Attend ONE social per quarter.
4. Attend TWO Outreach programs per year.
5. Identify and participate in ONE ministry for the year in Section I.
6. Lead OR Join ONE Committee planning a Big Annual Programs in Section II.

such.
_________If I cannot attend, or if I’m running late, I agree to call Pastor Hayden or a Youth
Coach attending or in charge at the time.
_________I will, to the best of my abilities, serve as a Youth Coach at the Community Praise
SDA Church beside the Youth Pastor, Melvyn Hayden III, for the saving of young souls in the
kingdom of Jesus Christ.
